Durham Health is a member of the Pegasus PHO which provides funding and subsidised care and special programs for those who are enrolled patients. The aim of this funding is to benefit and meet the health care needs of the local community.

Benefits of enrolling with the practice PHO

  • You will be eligible for reduced Doctor fees including ACC visits
  • Funded (no cost) doctor appointments for children 13yrs and under
  • Reduced prescription charges
  • Subsidised sexual health for those that meet criteria 
  • Funded (no cost) palliative care visits
  • Funded (no cost) acute packages of care where you may need to receive extended treatment and observation within the practice e.g IV antibiotics for an infection
  • entry into all the screening programs associated with womens health
  • Funded (no cost) childhood immunisations
  • Funded (no cost) Before School checks for 4 year olds

How to enrol

To provide the best care for our patients, our practice has a maximum enrolment limit. When we reach full capacity, we must pause new patient enrolments until spaces become available.

When spaces do open, priority will be given to:

  • Patients who do not currently have a local GP
  • Patients who live or work in the Waimakariri area

Enrolment form downloads:

  • Please download and complete all the forms below, then email through to reception@durhamhealth.co.nz or drop them in to the practice. Alternatively you can call into Durham Health and collect forms from reception. 
  • Note that you will be enrolled with the medical centre and not with a specific doctor, though we do encourage you to develop a relationship with 1-2 doctors in the interest of continuity of care
  • Every person must have their own form filled out and all persons aged over 16yrs need to sign their own form.
