How to enrol
The easiest option is to click here and complete the online enrolment form
Alternatively, you can download and print a physical form and drop that into the clinic or send via email to reception@durhamhealth.co.nz . You can also call into Durham Health and collect forms from reception.
Please note:
- To provide the best care for our patients, our practice has a maximum enrolment limit. When we reach full capacity, we must pause new patient enrolments until spaces become available.
- When spaces are available, priority will be given to:
- Patients who do not currently have a local GP
- Patients who live or work in the Waimakariri area
- Note that you will be enrolled with the medical centre and not with a specific doctor. However we do encourage you to develop a relationship with 1-2 doctors in the interest of continuity of care
- Every person must have their own form filled out and all persons aged over 16yrs need to sign their own form.
