Durham Health is a member of the Christchurch PHO which provides funding and subsidised care for those who are enrolled patients. The aim of this funding is to benefit and meet the health care needs of the local community.

How to enrol

Option 2: you can download and print the form below. Once completed you can drop into reception or send via email to reception@durhamhealth.co.nz .

Option 3: you can pick up enrolment forms from reception

Please note: 

  • To provide the best care for our patients, our practice has a maximum enrolment limit. When we reach full capacity, we must pause new patient enrolments until spaces become available.
  • When spaces are available, priority will be given to:
    • Patients who live in the Waimakariri area, and who do not already have a local GP
  • Note that you will be enrolled with the medical centre and not with a specific doctor. However, we do encourage you to develop a relationship with 1-2 doctors in the interest of continuity of care
  • Every person must have their own form filled out and all persons aged over 16yrs need to sign their own form.

  • Funded (no cost) GP appointments for children 13yrs and under
  • Reduced prescription charges
  • Funded sexual health consults for those that meet criteria

  • Funded (no cost) acute packages of care where you may need to receive extended treatment and observation within the practice e.g IV antibiotics for an infection
  • Entry into all the screening programs associated with womens health
